Program Description:

Any Baby Can case management program is a 9-12 month comprehensive support service designed to build a stable family foundation in order to increase the quality of life for the journey ahead. Case managers work to strengthen the family unit in order to increase the success of the client. Services and individual long-term goals are tailored to client.

Our mission is to serve families with children facing serious health or developmental challenges.

0-12

Who is eligible to receive services through this program?

Families who have a child birth through 17 years old that have a chronic illness, disability, developmental delay, health risk or challenge that live in Bexar, Atascosa, Bandera, Blanco, Caldwell, Comal, Edwards, Frio, Gillespie, Guadalupe, Hays, Kendall, Kerr, Medina, Real, Uvalde, or Wilson county.

Where does this program provide services?

Services are provided in the home or office as requested by the family.

Do clients pay for services?

No
